Primary Care and Population Health Grand Rounds (Time and Location Varies)

At the end of this continuing medical education activity, participants should be able to:

  1. Identify current models of care that transform primary care delivery;
  2. Describe and explain the role of patient reported data and use of data tools to better inform patient care;
  3. Discuss the design and implications of health policy and how it might impact local practice; and explain the “levels” of intervention and the “levels” to enact change.
