The applicant should be a phycian between the ages of 40 and 65 years, and must be a United States or Canadian citizen. He or she should preferably be a practicing physician, with a distinguished career in teaching and/or medical research at the DHMC. A physician who has been a recognized specialist in a well defined field of medicine.

(Not to exceed 2 pages)

  • A description of the candidate's commitment to primary prevention and health promotion.
  • A description of immediate and long-term objectives of the candidate, explaining how the award will contribute to their attainment.
  • Curricula vitae of the applicant and key personnel. Do not exceed 3 pages each. Please limit publications to the past 5 years.
