In order to receive Patient Safety Training Center (PSTC) backdoor badge access, please watch the following video and then complete the forms below it.

The following forms must be completed in order to receive PSTC backdoor badge access:

• PSTC Badge Access Orientation Video Attestation form
• PSTC at Dartmouth-Hitchcock Ground Rules for Participation form
• SBER Media Consent and Release Authorization form

Once these forms are complete, please email davis.w.mcgraw@hitchcock.org to request PSTC badge access.
