
Physician Group Practice Demonstration Project
The two year demonstration project, which began January 1, 2011, will study whether improving care in a proactive and coordinated manner also reduces costs.
Dartmouth-Hitchcock will apply care management practices and programs used with commercial managed care enrollees to Medicare fee-for-service beneficiaries. Disease management initiatives will focus on patient outreach and education surrounding preventive care services and follow up for targeted chronic disease programs. Disease management and case management programs will focus on congestive heart failure, diabetes, coronary artery disease, hypertension, advanced pulmonary disease, and on beneficiaries with high cost or complex medical conditions.
